An Occupational Therapist will provide an assessment report of your health and social care needs. The aim of the assessment is to promote wellbeing in accordance with section1(2) of the Care Act 2014. This assessment will not only focus on your needs but also take into consideration the needs of your carer if one is identified. This report will usually be completed within your home. The price also includes travel and administration time to produce the report, which outlines the recommendations and the care and support plan.
