Assessment
We aim to carry out our assessment within two weeks of referral being received usually in the clients home or an assessment centre. The assessment can last up to two hours.

Treatment
The treatment phase is discussed and agreed. The outcomes are set and are carried out within 3 months.

Review
After the treatment phase we review the outcomes and the goals.
Standard 4
The review will take place no later than a week after the treatment phase.
Discharged
Clients are discharged once they are satisfied that their goals have been achieved

Letter sent to you and your GP if required summarising the work completed. We encourage all of our clients to give feedback about the service you have received.

A report providing a summary of the work undertaken is sent to you and your GP if you require within two weeks of you being discharged from our service. We encourage all of our clients to give feedback about the service they have received.
Follow Up
We contact you within six months to check on how you are doing. All adaptation work is guaranteed up to a year.
